Facilities Request Form

Pandemic Update: Private and civic groups willing to cooperate fully with diocesan and civil mandates pertaining to the pandemic may begin reserving parish facilities for events occurring on or after September 1st of this year.


Please complete and submit the attached form for any future events that you would like to hold at a Saint Mary Parish facility. If you have any questions about the form or the policy, then please feel free to contact our Parish Office (482-2875) during regular office hours, and a member of our staff will be happy to assist you.

Please check the Facilities Calendar and allow two weeks to confirm that your event is scheduled correctly, and please contact our facilities supervisor at least three business days prior to your event to arrange access to the reserved facility. Again form must be submitted two (2) weeks in advance with the appropriate insurance and deposits when applicable.

Note: a team of volunteers provides live-streaming of a limited number of parish events, but this live-streaming ministry is NOT available upon request.
